THE APPLICATION MUST BE COMPLETED AND SIGNED BY BOTH THE LICENSEE AND THE PROPOSED AIDE MUST BE SUBMITTED WITH THE $10.00 APPLICATION FEE BEFORE THE APPLICATION WILL BE CONSIDERED.

The following information is required on the application:

Each duty listed under unit A should have corresponding information included under B. Project the amount of hours proposed for duty listed under A 1 in unit B 1. Please be as specific as possible so that the review process may be expedited and will not result in a return of the application for additional information.

PLEASE NOTE: If you wish to register someone as both a speech-language pathology and audiology aide, we will require two separate registration forms and the applicable fee for each registration. Each registration must be specific to the discipline for which you are registering the paraprofessional and must clearly define the appropriate duties for either speech-language pathology
