Required Professional Experience Packet

(US GRADUATES)

THE FOLLOWING ITEMS MUST BE SUBMITTED AND APPROVED BY THE BOARD PRIOR TO BEGINNING THE RPE.

• Application for Temporary Required Professional Experience Licensure
• $35 registration fee (payable to S.L.P.A.B.)
• RPE Temporary License Acknowledgement Statement (signature page only)
• Page 2 of the Live Scan form completed by the live scan operator. (You must be fingerprinted specifically for this Board).
  (Fingerprints must be cleared through Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI) before temporary license may be issued)

THE REMAINING ITEMS LISTED BELOW SHOULD BE SUBMITTED WITHIN THE FIRST 90 DAYS OF BEGINNING THE RPE. LATE SUBMISSION MAY RESULT IN A DELAY IN ISSUING THE PERMANENT LICENSE AND MAY REQUIRE YOU TO CEASE PRACTICE UNTIL THE PERMANENT LICENSE HAS BEEN ISSUED:

• Application for Licensure
• $25 initial license fee (payable to S.L.P.A.B.)
• "Official" graduate and undergraduate transcripts (sent directly from the university)
• Copy of Master's Degree
• Report of Clinical Practicum (on our form and must be mailed directly from the University to our office)
  
  • Speech-Language Pathology
  • Audiology
• NTE scores in the field for which licensure is sought, with a passing score of 600 or higher sent directly from the examination service to the Board office.
• School calendar (if working in a public school setting)
• Upon completion of your RPE, the Required Professional Experience Verification form must be completed and submitted within 10 working days.
