Required Professional Experience Packet
(US GRADUATES)
THE FOLLOWING ITEMS MUST BE SUBMITTED AND APPROVED BY THE BOARD
PRIOR TO BEGINNING THE RPE.
THE REMAINING ITEMS LISTED BELOW SHOULD BE SUBMITTED WITHIN THE FIRST 90 DAYS OF BEGINNING THE RPE. LATE SUBMISSION MAY RESULT IN A DELAY IN ISSUING THE PERMANENT LICENSE AND MAY REQUIRE YOU TO CEASE PRACTICE UNTIL THE PERMANENT LICENSE HAS BEEN ISSUED:
- Application for Licensure
- $25 initial license fee (payable to S.L.P.A.B.)
- "Official" graduate and undergraduate transcripts (sent
directly from the university)
- Copy of Master's Degree
- Report of Clinical Practicum (on our form and must be mailed directly
from the University to our office)
- NTE scores in the field for which licensure is
sought, with a passing score of 600 or higher sent directly from the
examination service to the Board office.
- School calendar for each school year. (if working in a public school setting)
- Upon completion of your RPE, the
Required Professional Experience Verification form must
be completed and submitted within 10 working days. If you are working in a public school you will be required to submit a separate verification form for each school year. You must also provide a calendar for each school year. If you work during the summer you will be required to submit a separate verification form for the summer session. You will also be required to provide a letter from the school district that defines the dates and hours of the actual summer school session.

WE REQUIRE ORIGINAL SIGNATURES ON ALL FORMS AND APPLICATIONS. IF YOU REQUIRE VERIFICATION THAT A DOCUMENT HAS BEEN RECEIVED YOU SHOULD USE CERTIFIED MAIL, AND REQUEST A SIGNED RECEIPT.
