Reinstatement of a License

If you previously held a California license and would like to reapply for licensure you have two (2) options.

Option 1

You submit the following:

• Application for Licensure
• Fingerprints
• You must be fingerprinted specifically for this Board.
• You must retake and pass the Praxis Exam.
• You must include your previous license number however; you will be issued a new license number if your application is approved.

Option 2

If your ASHA certification was reinstated you may not qualify for this option.

If you hold current ASHA certification you may reapply using the Equivalent Qualifications Packet. You must include your previous license number however; you will be issued a new license number if your application is approved.
