Checklist of Items Required for Speech-Language Pathology or Audiology Licensure By Equivalent Qualifications

• Application for Licensure (Equivalent Qualifications)
  (must be typed or printed in blue ink)
• $60 application fee (made payable to SLPAB)
• Original "Verification of Certification" letter sent directly from ASHA.
• Fingerprint Card (FD 258) and $51 ($32 DOJ and $19 FBI) fingerprint processing fee
  (DOJ/FBI processing time is 60 to 180 days.)
  - OR -
• Page 2 of the Live Scan form completed by the live scan operator.
  (DOJ/FBI processing time is 72 hours to 30 business days.)
  Fingerprints must be cleared through Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI) before the license may be issued.

ADDITIONAL FORMS:

• Required Professional Experience Verification form
  This form used ONLY if you completed your CFY/RPE in California or outside the United States. You will also be required to provide a Verification of Certification Letter for the supervisor completing this form.
