Regular Packet - Applicants licensed in another state

Since you have already completed your required professional experience but do not have ASHA certification and are applying for licensure, you will need to complete the Application for Licensure form, Required Professional Experience (Verification) form, Report of Clinical Practicum form, and Fingerprint Card. With these completed forms you will need to provide a check for $116 ($60 for the license application fee, and $56 for the fingerprint-processing fee). In addition, you will need to provide official transcripts from your graduate and undergraduate schools, an original NTE exam score report, and the Verification of Licensure form completed and signed by every state board in which you hold a current license.

If circumstances prevent you from completing the supervisory section of the Verification form, you may request an exemption of this requirement. Individual’s who hold a current unrestricted license in another state or territory and have been practicing full-time (a minimum of 30 hours per week) for one full year within the past three years may qualify for this exemption. You must include a letter requesting an exemption from the RPE Verification requirements, include a current resume and verification of employment from your employer.

If you are also applying for a temporary license to practice in California while your application is being processed, you should complete the Application for Temporary License, submit a Letter of Good Standing from the state where you are licensed and an additional $30 temporary license fee. If it has been more than 5 years since you have taken and passed the NTE examination, you may also want to submit a Request for Waiver of the NTE Exam form, along with the required documents.
